Cost per visit $11.00 for the 1st copy and $2.00 each additional copy

• Must show proof of identification
• Must be born at Loyola hospital “ Foster G Mc Gaw”
• All births are originals
• We accept credit, debit, cash, checks, tap and pay.
  

Death certificate

Cost per visit $13.00 for the 1st copy and $4.00 each additional copy
All veterans are entitled to one free certified copy
